Life Aid helps seniors, disabled adults, veterans, and medically vulnerable individuals access free medical equipment, benefits and application assistance, care coordination, assisted living and skilled nursing placement support, home health and hospice referrals, food assistance, transportation resources, and hospital discharge support. Life Aid is also listed with 211 as a community resource, helping individuals and families connect with trusted support services throughout Los Angeles County. Our assistance is provided at no cost.
How We Help
Comprehensive support for seniors, disabled adults, veterans, and medically vulnerable individuals, from medical equipment and benefits navigation to care coordination, placement assistance, food resources, transportation, and hospital-to-home support.
Life Aid provides free medical equipment to seniors, disabled adults, and medically vulnerable individuals throughout Los Angeles County. Available equipment may include hospital beds, wheelchairs, walkers, mobility aids, and other durable medical equipment (DME), based on availability.
Life Aid accepts gently used medical equipment and supplies from individuals, families, and organizations. Donated wheelchairs, hospital beds, walkers, mobility equipment, and other usable items are redistributed to seniors and disabled adults in need throughout Los Angeles County.
Life Aid helps seniors, disabled adults, veterans, and medically vulnerable individuals understand and apply for essential benefit programs and community resources. Assistance may include Medi-Cal, Medicare, CalFresh, SSI, transportation programs, and other public or nonprofit support services.

Support may include assisted living placement, skilled nursing placement, home health and hospice referrals, hospital discharge planning, care transitions, and coordination with providers and case managers.

Care Coordination Support
Life Aid helps seniors, disabled adults, veterans, LGBTQ+ seniors, and families navigate complex care needs. We provide care coordination, benefits and application assistance, hospital discharge support, assisted living and skilled nursing placement guidance, and referrals to home health, hospice, transportation, food, medical equipment, and other community resources.

Food Programs
Providing food assistance and connecting seniors, disabled adults, veterans, LGBTQ+ seniors, and medically vulnerable individuals with fresh food, groceries, emergency food resources, and community food programs.
Medical Equipment (DME)
Providing access to free durable medical equipment (DME), including wheelchairs, hospital beds, walkers, mobility aids, and other essential equipment for seniors and disabled adults, based on availability.
Incontinence Supplies
Providing access to incontinence supplies and personal care products for seniors, disabled adults, and medically vulnerable individuals, based on availability and individual need.
Care Placements
Helping seniors and families navigate assisted living, independent living, congregate living, and skilled nursing placement options based on individual care needs and preferences.
Home Health, Hospice & Palliative Care
Helping seniors and families connect with appropriate home health, hospice, and palliative care providers based on individual needs, preferences, and available resources.
How Life Aid works
Life Aid helps seniors, disabled adults, veterans, LGBTQ+ seniors, medically vulnerable individuals, and families navigate available care and community resources. We assess individual needs, explain available options, and provide no-cost assistance with medical equipment, benefits applications, care coordination, placement support, referrals, and other essential resources.

FOR HOSPITALS, CASE MANAGERS & CARE PROFESSIONALS
Life Aid works with hospitals, case managers, discharge planners, social workers, physicians, and other care professionals to help seniors and disabled adults access needed resources after hospitalization or during care transitions. We assist with medical equipment, benefits applications, assisted living and skilled nursing placement support, home health and hospice referrals, transportation, food assistance, and other community resources. Our assistance is provided at no cost, and individuals retain full freedom of choice when selecting providers and services.

Our Promise
Life Aid provides assistance at no cost and respects every individual’s right to choose the services and providers that are best for them. We do not require the use of any specific home health, hospice, assisted living, skilled nursing, or other provider. Our role is to provide unbiased guidance, care coordination, referrals, and access to community resources while protecting each person’s freedom of choice.
