CARE COORDINATION & PLACEMENT SUPPORT

FIND THE RIGHT CARE & PLACEMENT SUPPORT

Life Aid helps seniors, medically vulnerable adults, individuals with disabilities, and families navigate complex care decisions. We provide personalized care coordination, placement assistance, and referrals to appropriate assisted living communities, home health agencies, hospice providers, and other supportive care resources based on each person’s individual needs.

CARE OPTIONS WE HELP WITH

Life Aid helps individuals and families explore appropriate care options based on medical needs, level of support, personal preferences, location, and available resources. Depending on the situation, we may assist with placement, referrals, care coordination, and connections to licensed providers and community-based services.

ASSISTED LIVING PLACEMENT

We help seniors and families identify appropriate assisted living communities based on care needs, location, preferences, and available resources. Our team assists with placement coordination, communicates with communities, and helps families understand available options before making a decision.

HOME HEALTH CARE REFERRALS

We help individuals and families connect with appropriate licensed home health agencies when in-home skilled care may be needed. Our team assists with referrals, care coordination, and communication with providers based on each person’s needs and available services.

HOSPICE CARE REFERRALS

We help individuals and families connect with appropriate licensed hospice providers when comfort-focused end-of-life care may be needed. Our team assists with referrals, care coordination, and communication with providers to help families understand available hospice care options and next steps.

HOW OUR PLACEMENT & REFERRAL PROCESS WORKS

Our process begins with a personalized assessment of each individual’s care needs, preferences, location, and available resources. We coordinate with families, hospital discharge planners, social workers, physicians, and community providers when appropriate to identify suitable care options. Our team then assists with referrals, placement coordination, documentation, and follow-up to help support a smooth transition to the appropriate level of care.

WHO WE SUPPORT

Life Aid supports seniors and older adults, medically vulnerable individuals, adults with disabilities, families and caregivers, and individuals transitioning from hospitals or skilled nursing facilities who need help identifying appropriate care, placement, and community resources.
